3 Easy Steps:
- Visit our home. Call and make arrangements with Lee for a private tour of our facility. At this time you can determine if your needs can be met.
- Request a nursing assessment to determine if Green Meadows feels confident they can meet your needs. At this time an Lee Kelly RN will come to your home and spend approximately 1hour completing the assessment form. This assessment is free of charge. A small mileage fee may be applicable if traveling a distance over 50 km.
- Consent is given to obtain a medical history from your family doctor.
Fees
Long term care with 24/7 staff – $216.00 / day
